为探究二维剪切波弹性成像中肝硬度值(liver stiffness measure, LSM)及生理参数与慢性肝病纤维化程度的关联性在纤维化程度评估中的相对指导作用,本研究自首都医科大学附属北京地坛医院获取88例慢性肝病患者,均行肝脏组织穿刺和超声弹性成像检查。采用受试者工作特征(receiver operating characteristic,ROC)曲线评估LSM值对肝纤维化的分级效能,同时和临床血清学诊断天冬氨酸氨基转移酶和血小板比率指数(aspartate amino transferase-to-platelet ratio index, APRI)及4因子纤维化指数(fibrosis4score, FIB-4)进行比较。以肝活检结果为金标准,LSM值对肝硬化(=F4)的ROC曲线下面积(area under ROC curve, AUC)值为0.90(95%CI:0.82~0.98),进展期肝纤维化(≥F3)AUC值为0.85(95%CI:0.74~0.97),显著性肝纤维化(≥F2)AUC值为0.88(95%CI 0.80~0.96),均高于APRI和FIB-4。二维剪切波弹性超声LSM值较生理参数APRI和FIB-4对肝纤维化分期更有指向性。

To explore the guiding role of the correlation between liver stiffness measure (LSM) and physiological parameters in two-dimensional shear wave elastography with the fibrosis degree of chronic liver disease(CLD) in the evaluation of fibrosis degree,88 CLD patients underwent both liver biopsy and 2D-SWE in Ditan Hospital were included. ROC curves were used to evaluate the diagnostic performance of each previously mentioned parameter measure on the classification of liver fibrosis in CLD. Liver biopsy result was used as the gold standard.The AUC values of LSM for cirrhosis, advanced fibrosis, and significant fibrosis are 0.90 (95%CI:0.82~0.98), 0.85(95%CI:0.74~0.97),0.88 (95%CI:0.80~0.96)relatively. They are all higher than the performance of APRI and FIB-4.Comparing to the physiological parameters, 2D-SWE is more indicative to the liver fibrosis staging.
