为探究不同固定参数对青少年双下肢不等长髓内钉固定术后股骨稳定性的影响,本研究利用MIMICS软件建立有骺板的青少年股骨模型,使用SolidWorks软件建立含4种螺钉锁定方式、2种钉体长度(200 mm、300 mm)、2种钉体直径(8.5 mm、9.5 mm)及2种钉体材料(不锈钢、钛合金)的8组髓内钉股骨模型。将模型导入ANSYS Workbench软件施加髋关节和肌肉载荷,进行仿真测试,分析股骨的位移及股骨和髓内钉的应力,之后使用SPSS 25软件对股骨应力、位移及髓内钉应力结果进行分析,以确定各因素对髓内钉固定股骨时稳定性影响的显著性。研究表明,髓内钉钉体在长度大、直径小的情况下股骨位移小,具有较好的稳定性;不锈钢材料可降低股骨应力、稳定性好,但其应力遮蔽效应明显;近端锁定螺钉成角度与水平布局相结合,可有效降低股骨及植入物应力,增加稳定性。本研究可有效评估髓内钉不同参数对术后股骨稳定性的影响,为青少年髓内钉的参数优化设计提供参考,提高髓内钉在手术使用过程中的安全性和有效性。
Abstract:
To investigate the impact of different fixation parameters of intramedullary nail on femoral stability after internal fixation in adolescents′ leg length discrepancy, we establised an adolescent femur model with epiphyseal plate by MIMICS software. 4 kinds of locking methods and 8 groups of intramedullary nail femoral models including 2 kinds of nail lengths (200 mm, 300 mm), 2 kinds of nail diameters (8.5 mm, 9.5 mm) and 2 kinds of nail materials (Stainless steel, Titanium alloy) were established by SolidWorks software. The hip joint and muscle loads were conducted simulation test by importing the model into ANSYS Workbench to analyze the displacement of femur and the stress of femur and intramedullary nail. SPSS 25 software was used to analyze the stress, displacement of femur and stress of intramedullary nail, so as to determine the significance of the influence of various factors on the stability of intramedullary nail fixation of the femur. The results showed that the value of femoral displacement was small and the stability was good when the length of intramedullary nail was large and the diameter was small. Stainless steel material reduced the stress of femur and behaved high stability, but the stress shielding effect was obvious. The combination of oblique and horizontal placement of the proximal locking screw effectively reduced the stress of femur and increased stability of the femur and implant. The model can effectively evaluate the influence of different parameters of intramedullary nail on postoperative stability of femur, and provide reference for the parameter optimization design of the intramedullary nail for adolescents, so as to improve the safety and effectiveness of intramedullary nailing in operation.
